Transition Programs and Resources

  • Welcome to the Carbon Lehigh Transition Resources. The purpose of this site is to assist families, youth, educators, and agencies with identifying varying resources while navigating the process of transition from school age into adult life.

    You will find resources that support youth and young adult ages 14-21. However, many of these resources can be utilized for students younger than age 14.

    The school districts and career and technical schools served are: Allentown, Carbon Career Technical Institute, Catasauqua, East Penn, Jim Thorpe, Lehigh Career Technical Institute, Lehighton, Northern Lehigh, Northwestern Lehigh, Palmerton, Panther Valley, Parkland, Salisbury Township, Southern Lehigh and Weatherly.

    Carbon Lehigh Transition Coordinating Council (CLTCC)

    The Mission of CLTCC is to promote effective transition services for students with disabilities, their families, educators, and community service providers from school to adult community life. The meetings consist of presentations geared towards transition age students, networking time and resource sharing regarding transition. We are a group of parents, agencies, employers, educators from the Intermediate unit and our 14 school districts, Carbon and Lehigh vocational technical school and Higher Education in the area.
